Single choice
Anonymous – you will not be identified and will not be able to track the investigation of the complaint.
Confidential – your identity will be protected and you will receive information about the investigation of your complaint.
Public – you will be identified and the accused will have access to your name. You will receive information about the complaint.

Fill in the identification fields only in cases of 'Confidential' or 'Public' complaints.
